Fields, Jack Earl, Jr.--born August 5, 1938 in Tulsa, OK--passed away September 14, 2005 in Tulsa, OK. Survived by: his wife of 43 years, JoAnn Fields; his 5 children Jeff, Rhonda, Jim, Joe and Laura; 9 grandchildren; and brother John Fields. Jack had a deep love for his family and friends. He lived up to his name and was truly a "Jack of all trades". He was a kind and gentle man and will be missed by all his family and friends. We love you.
