Forman, Pam Simons, 53, died Friday after a long illness in Tulsa. She is survived by her common law husband, Mark Brown; a daughter, Suzette Forman Pearson of Tulsa; a son, Patrick Douglas Forman of Boca Raton, FL; two brothers, Geoffrey Simons of Boca Raton, FL and Michael Simons of Mesa, AZ; twin granddaughters, Raegan and Jordan Forman of Shawnee, OK. Her parents, George and Sally Poe Simons of Okmulgee, OK preceded her in death. A memorial service for Pam Forman will be held 11:00 AM Tuesday, November 8, 2005 at Trinity Episcopal Church in Tulsa with Father Joseph Alsey officiating.
